Our Office Policies
 

Non-OHIP Patients


 

We current accept the following substitutes for OHIP cards: UHIP, IFHP (Interim Federal Health Program) or a valid Health Insurance Card from another province (exception Quebec).  With our apologies, for patients with a Quebec Health card, you will be charged cash for your appointment and provided with a medical receipt.  It is your responsibility to contact your provincial government to obtain a refund.

If you do not have the above items available, you will be required to pay cash for your consultation or follow-ups prior to being seen.

Cancellations


 

We require a 24 hour cancellation notice if you are unable to make your scheduled appointment.  Should you need to cancel, we will do our best to reschedule you at our nearest opening.  If you do not make your scheduled appointment, and fail to give us notice, you will be charged a rebooking fee.

Clinic Hours


 

Hours of Operation are Monday to Thursday 9-4 pm.  We are closed for lunch between 12-1.

New Patients


 

We require a valid OHIP card for all appointments.  To obtain a valid health card, please see this website to find your nearest Service Ontario office.  For your information, there happens to be a location across the street from our office (777 Bay Street - see link below). Please note we require two things prior to new patient consultations being booked.  The first is a referral note from a physician in Ontario with valid billing number, address, and fax number.  The second is patient information, including a valid OHIP card with version code, DOB, address, and contact phone number.  New appointments will not be booked until the above information is received in full.
